Gear can be upgraded from +1 to +15 using Upgrade Runes, raising Attack, Defense, Armor and Hit Chance by about 1% per level and unlocking bonus stat slots at the top end. Failure never destroys an item — at worst it drops one level (and +0 to +4 lose nothing at all).
Four rune grades each cap at a different level and add a flat success bonus. Right-click a rune and pick your gear to attempt an upgrade.

Bonus stat slots: gear carries 1 attribute slot up to +12, a 2nd at +13, and a 3rd at +15 — each granting a random skill bonus from your vocation's pool.
The Rune Forge tab in the Item Fusion panel lets you see your exact success and failure odds first, then optionally spend gold or Tibia Coins to boost your success chance before you commit. Zero fee = the same rate as a normal right-click upgrade, so it's a pure de-risk option, not a gate.
